In the topmost column, your current 2025 voter list name, EPIC number, address, part number, serial number, assembly constituency and state name will be printed. You only need to paste your current passport-size photograph next to your voter card photo.
Everyone must fill up the A column section. For the B column, those who have their own names in the voter list after the 2002 'SIR' will fill it up. Information, including father's or mother's name and other details, must be provided according to the list existing in the 2002 list.
For the C column, those whose parents' names were in the 2002 list will fill up the form accordingly. Even if parents are deceased, write all information in the column according to the 2002 list. If parents' names are not there, then fill up with grandfather-grandmother's names and information existing in the list.
Those who had their own and their parents' names in the 2002 list will fill up both columns. Those who did not have names in the 2002 list, but had parents', grandfather-grandmother's names, will not fill up the B column accordingly, but will fill up the C column.
Finally, the applicant must sign themselves. If an enumeration form has come in the name of any family member, but they are staying outside, then any adult member of the house can fill up and submit the form. In that case, the adult member will have to sign the form and write their relationship with the applicant.
You can take help from BLO and any political party's BLAs for filling up the form. If you have any doubt, question, or complaint, you can call the commission's toll-free number 1800-11-1950 from 8 AM to 8 PM.

Don't panic about spelling mistakes of names, wrong surnames in the 2002 list. If your name or parents' names are not in the 2002 list, keep the commission's designated 11 documents (keep Aadhaar only as identity proof or if anyone from your family member's names is in Bihar's post-'SIR' list, keep that handy).
