Applicants seeking benefits under the West Bengal government's Annapurna Yojana will need to provide detailed information about their family, income, assets, education and banking details. The application form spans 12 pages and requires information about every member of the household.
To make the process easier, here is a page-wise guide explaining what needs to be filled in each section of the form.
Page 1: Family identification
Enter the name of the Head of Family (HOF) as mentioned on the ration card. The head does not necessarily have to be a woman.
Mention the date of birth.
Tick the appropriate gender option (Male/Female/Other).
Enter the Aadhaar number.
Provide the Family ID from the digital ration card, if available.
Mention the total number of family members.
Write the complete residential address.
Provide a mobile phone number.
Enter the name, date of birth, gender, relationship with the head of the family, and Aadhaar number of each family member.
Indicate with āYesā or āNoā whether the member(s) are applying for this financial assistance.

Page 2: Bank account details
Provide the bank name of the head of the family and all adult members.
Enter the bank account number.
Mention the IFSC code.
Details of an active bank account are necessary for receiving benefits through Direct Benefit Transfer (DBT).
Page 3: Voter ID, caste and ration card information
Enter the EPIC number (Voter ID number) of the head of the family and all adult members.
Select the applicable category: General, SC, ST, OBC, EWS, or Others.
Mention whether the family has a digital ration card.
Specify the ration card category (AAY, PHH, RKSY-I, RKSY-II, etc.).
State whether the family regularly collects ration.
Indicate whether the family owns three or more permanent (pucca) rooms.
Page 4: Land, vehicle and health insurance details
Mention the total amount of land owned by the family.
State whether any family member owns a four-wheeler vehicle.
If the family has health insurance, specify whether it is government-sponsored or private.
Provide the sum assured and other insurance details.
Enter the PAN number, if available.

Pages 5 and 6: Employment and educational qualifications
Mention whether family members are engaged in government service, private jobs, business, farming, labor work, unemployed status, or any other occupation.
Provide the educational qualifications of each family member.
Mention the number of educated members in the family.
Specify the highest level of education attained by each member.
Page 7: Income and other information
State whether any family member is a public representative such as an MP, MLA, Panchayat member, etc.
Mention whether anyone receives a government pension.
Indicate whether the family has GST registration.
Provide the total annual family income.
Pages 8 and 9: Special identification and student details
Provide details of any government-issued cards such as CAA, KCC, Artisan Credit Card, MCC, or similar cards, if applicable.
Mention if any SIR-related case is pending.
Enter the names of school-going students, their class, school name, and type of school.

Page 10: Vaccination and government schemes
State whether children's vaccination schedules have been completed.
Mention if any family member is receiving benefits from other government schemes and provide the scheme names.
Pages 11 and 12: Declaration and signature
Declare that all information provided in the form is correct.
The head of the family must sign in the designated space.
Since incorrect information may lead to the cancellation of government benefits, applicants are advised to verify all details before submission.
