A fatal bungee jumping accident in Brazil has triggered an investigation after a 21-year-old woman died during what authorities believe may have been a serious safety failure at an adventure sports facility.
Maria Eduarda Rodrigues de Freitas lost her life during a jump from the Skeleton Bridge in Limeira, a municipality in São Paulo state. The incident occurred on Saturday and has since drawn widespread attention after videos from the site began circulating on social media.
According to witness accounts, the safety cord that should have secured the participant was allegedly not attached before the jump took place.
Videos capture moments before the fatal fall
The footage shared online reportedly shows members of the bungee jumping crew escorting Maria to the edge of the bridge before the jump.
Witnesses later pointed out that the primary safety rope appeared to remain on the platform rather than being connected to the participant. Moments after the jump, people gathered at the site can reportedly be heard shouting in alarm after realising something had gone wrong.
Local media reports stated that the fall was from a height of approximately 130 feet.
Emergency responders rushed to the scene following the accident, but the young woman was declared dead before she could receive further medical assistance.

Authorities launch investigation
Hindustan Times noted that Brazilian authorities have opened an investigation to determine exactly how the incident occurred and whether established safety protocols were followed.
Police recovered the body and transferred it to the Legal Medical Institute for examination as part of the ongoing inquiry.
According to reports, six individuals connected to the operation were detained for questioning following the incident. Investigators are examining whether criminal negligence or other offences may have contributed to the tragedy.
Officials have not yet announced any formal charges as the investigation continues.

Social media posts reveal final moments before jump
Hindustan Times reported that hours before the accident, Maria reportedly shared images and videos from the adventure sports venue on social media.
The posts included photographs of her entry wristbands, images of the bridge and pictures showing staff members involved in organising the activity. In one post, she reportedly joked about preparing to jump from the bridge.
The company responsible for operating the bungee jumping experience has also come under scrutiny as investigators examine its safety procedures and operational practices.
The incident has renewed discussions about safety standards in adventure tourism and extreme sports activities, particularly regarding equipment checks and supervision before participants undertake high-risk experiences.
